Influence of anterior thalamic nucleus deep brain stimulation on cognitive ability of epilepsy patients and relevant factor analysis

Abstract:Objective To explore the influence of anterior thalamic nucleus deep brain stimulation (ANT-DBS) on cognitive ability of epilepsy patients and the relevant factors. Method Clinical data of 15 epilepsy patients undergoing ANT-DBS were analyzed respectively. The McHugh classification was uesd to evaluate the surgical outcome, the Wechsler intelligence scale (WAIS-Ⅳ) score , Montreal cognitive assessment ( MoCA ) and Center for epidemiological survey depression scale ( CES-D ) were used to evaluate the cognition and emotion before and after surgery. The factors related to cognitive ability were analyzed. Results Fifteen patients were followed up for 1 to 1.5 years, satisfactory results of controlling seizure was achieved in 4 patients and non-satisfactory results in 11. The postoperative visuospatial and executive function, naming, memory, attention, language, abstraction, orientation and memory quotient were higher than preoperative results (P<0.05). The cognitive improvement was positively correlated with the onset age (r=0.427, P=0.031), negatively correlated with course of disease (r=-0.723, P=0.002) and frequency of seizure (r=-0.315, P=0.040). There was no significant correlation between cognition and seizure reduction, seizure types and drug categories (P> 0.05). Conclusion ANT-DBS can obviously improve the cognitive ability of epilepsy patients, thus being considered as a treatment mean of epilepsy, especially for the intractable epilepsy patients without surgical indication.
