Therapeutic effect of cube directional dual-soft channel with different doses of urokinase for hypertensive cerebral hemorrhage

Abstract:Objective To observe the therapeutic effect of cube directional dual-soft channel with different doses of urokinase for hypertensive cerebral hemorrhage (HCH).Methods Among 163 HCH patients,single-soft channel drainage (SCD) was performed in 42 patients with hematoma volume less than 50 ml,and dual-soft channel drainage (DCD) in 12l patients with hematoma volume equal or more than 50 ml.Then,the patients in DCD group were randomly divided into three subgroups:small,medium and large dose groups,and 10 000,30 000,50 000 U urokinase were injected in the three subgroups respectively.The therapeutic effect was compared between SCD group and both DCD group and the three subgroups in DCD group.Results There were no significant differences in the rate of good recovery 2 weeks after surgery,life-independent rate 6 months after surgery,the hematoma evacuation rate 7 d after surgery and the recurrence rate of hematoma between SCD group and DCD group (P > 0.05).In DCD group,the hematoma evacuation rate was much higher in large dose group than in small and medium dose groups 3,7 d after surgery (P< 0.05),but there was no significant difference in the recurrence rate ofhematoma between the three subgroups (P > 0.05).The intracranial infection rate,extubation time and hospital stay in DCD group were less than those in SCD group (P < 0.05),and extubation time and hospital stay in large dose group were less than those in small and medium dose groups (P< 0.05).Conclusion DCD is eutherapeutic for patients with a large hematoma,and 50 000 U urokinase can improve the evacuation rate ofhematoma without increasing the risk ofrebleeding.
