Diagnosis and treatment of pilocytic astrocytoma

Abstract:Objective To investigate the treatments of pilocytic astrocytoma. Methods The clinical data of 60 patients with pathologically-proved pilocytic astrocytoma were analyzed retrospectively. The locations of tumor were in the posterior fossa in 39 cases (including cerebellar vermis in 9, cerebellum 29, brainstem 1), spinal cord in 7, supratentorial cerebral lobes in 4, lateral ventricles in 4 and sellar region in 6. All patients underwent microsurgery. According to the tumor location, the appropriate surgical approach was chosen. Results Total tumor resection was performed in 56 cases and partial resection in 4. The patients with partial resection were treated with adjuvant radiotherapy or gamma knife radiosurgery. All the patients were followed up from 6 months to 10 years, and no recurrence was found in total resection patients. The partial resection patients were stable and the tumor had no significant increase in size. Conclusions For the pilocytic astrocytoma lesions, complete resection without adjuvant therapy can achieve satisfactory postoperative outcome. In the patients without complete resection, adjuvant radiotherapy or gamma knife treatment can also get excellent therapeutic outcome.
