Application of cortical electrical stimulation in the resection of epileptogenic foci in the eloquent area

Abstract:Objective To analyze the application of cortical electrical stimulation in the resection of epileptogenic foci in the eloquent area. Methods Clinical data of 8 patients with intractable epilepsy in the eloquent area were analyzed retrospectively. According to the video EEG and electrical stimulation results, the relationship between the epileptogenic focus and eloquent area was determined. The eloquent area was protected and epileptogenic focus was resected at a maximum level. Results The surgery was successful. There was no decrease of postoperative muscle strength in 2 patients with left frontal encephalomalacia whose preoperative muscle strength was grade 4. The partial visual field defect occurred in 1 patient with focal cortical dysplasia in right occipital lobe. All the patients were followed up for 6 months to 3 years, no seizures occurred in 6 patients and 90%reduction in seizures in 2. Engel grade I to II was in all the patients. Conclusions Cortical electrical stimulation is the gold standard for localization of the eloquent area. The application of cortical electrical stimulation in the treatment of epilepsy in the eloquent area can remove the epileptogenic focus to the utmost extent based on the preservation of eloquent area, which can improve the quality-of-life and get better control of epilepsy.
