Clinical evaluation of MRS combined with DWI for the grade and invasiveness of glioma

Abstract:Objective To explore the value of hydrogen-1 magnetic resonance spectroscopy (1H-MRS) combined with diffusion-weighted imaging (DWI) in judging the grade and invasiveness of glioma so as to provide imaging support for evaluating the biological characteristics of glioma and making surgical plan. Methods Clinical data of 27 patients with supratentorial glioma were analyzed retrospectively, who were examined by routine magnetic resonance imaging (MRI), MRS and DWI before operation. The differences in the ratios of choline (Cho) to N-acetylaspartate (NAA) and Cho to creatine (Cr), apparent diffusion coefficient (ADC) and Ki-67 labeling index (Ki-67 LI) between normal brain tissue, high grade and low grade gliomas were compared and the correlation between ADC and Ki-67 LI were analyzed. Results Compared with the normal brain tissue, the ratios of Cho to NAA and Cho to Cr increased and ADC decreased significantly in high-grade glioma (all P < 0.01). Compared with low-grade glioma, ratios of Cho to NAA and Cho to Cr increased, Ki-67 IL increased and ADC decreased significantly in high-grade glioma (all P < 0.05). Compared with the normal brain tissue, ratios of Cho to NAA and Cho to Cr increased markedly in low-grade glioma (both P < 0.05). The ADC and Ki-67 LI were negatively related to each other in high-grade glioma (r = - 0.683,P<0.05). There was no correlation between ADC and Ki-67 LI in low grade glioma (r = 0.187, P > 0.05). Conclusion The combination of H1-MRS and DWI can evaluate the malignancy and invasiveness of glioma, which is important for surgical treatment.
