Abstract:Venoarterial extracorporeal membrane oxygenation(V-A ECMO)has become increasingly prevalent in the provision of cardiopulmonary support for patients with cardiogenic shock.However,while improving systemic perfusion,V-A ECMO concurrently elevates left ventricular afterload,leading to ventricular distension and subsequently triggering a spectrum of adverse events.Despite the existence of numerous left ventricular unloading strategies designed to mitigate these risks,optimal management during V-A ECMO remains undefined due to a paucity of high-level randomized evidence.This review methodically analyses the therapeutic efficacy of these interventions and provides a concise summary of recent research advances.
