The incidence and risk factors of postoperative hypoxemia and their impact on prognosis in infants after cardiac surgery

Abstract:Objective To investigate the incidence and risk factors of postoperative hypoxemia and their impact on prognosis in infants after cardiac surgery.Methods The clinical data of 209 infants underwent cardiac surgery at the Department of Cardiology,Guangdong Provincial People's Hospital between January 1,2019 and April 30,2019 were retrospectively collected,and they were divided into hypoxic and non-hypoxic groups by whether the oxygenation index was less than 300mmHg.Curcurve relationship between oxygenation index and mechanical ventilation and length of hospital stay were analyzed using linear regression model combined with restricted cubic splines model,and independent risk factors for postoperative hypoxemia were analyzed by univariate and multivariate logistic regression.Results A total of 209 infants were enrolled in the study,with a postoperative hypoxemia incidence rate of 14.8%.Preoperative reduction in albumin levels and intraoperative blood transfusion emerged as independent risk factors for postoperative hypoxemia.The non-hypoxic group had a median mechanical ventilation duration of 21(7,74)hours,intensive care duration of 166(41.25,305.75)hours,and hospitalization duration of 8.05(5.9)days;whereas the hypoxic group exhibited a median mechanical ventilation duration of 73(46,139)hours,intensive care duration of 265(189,358.5)hours,and hospitalization duration of 12.2(9.3,16)days.The hypoxic group experienced significantly longer periods of mechanical ventilation,intensive care stay and hospitalization compared to the non-hypoxic group;however both groups were discharged alive.Conclusions Hypoxemia affects clinical outcomes,and provides reference for optimizing postoperative monitoring by identifying independent risk factors for postoperative hypoxemia.
