Study of platelet microRNA activation and platelet function in adult patients un-dergoing coronary artery bypass grafting

Abstract:[ Abstract]:Objective To evaluatethe change of platelet microRNA levels and the platelet functionin patients after off-pump versus on-pump coronary artery bypass graft ( CABG) surgery and to observe the relationship between the function of platelet and car?diopulmonary bypass ( CPB) time. Methods Consecutive adult patients were assigned to on-pump or off-pump CABG surgery from August 2015 to September 2015. Exclusion criteria included emergency operation, re-operation, compound operation and the chronic disease of liver, renal and blood system. Preoperative demographic data was recorded. The function of platelet ( MA) and coagulation were tested by thrombelastogram ( TEG) at two intervals, namely after anesthesia induction ( T1) and 5mins after protamine neutraliza?tion ( T2) . The expression levels of miR-223 and miR-126 were detected by real-time PCR. Results Fifty patients were entered into this study (25 patients off-pump and 25 on-pump CABG surgery). Compare with T1, the hemoglobin (Hb), platelet count (PLT) and MA levels in both groups significantly decreased after heparin reversal. The platelet function of most patients were at normal range. There was significant difference between the two groups in the reduction of the Hb and PLT except MA. The level of both microRNAs in two groups significantlyincreased at T2 ( P<0.001) . There was a liner correlation between MA and CPB ( r=0.64, P =0.008) . Con?clusion The levels of HB, PLT and MA in two groups significantly decreased after heparin reversal. The effect of on-pump CABG surgery on platelet function was similar to that of the off-pump surgery. The function of platelet after protamine administeration in?creased with CPB time, the pattern of which was different from the trend of platelet microRNAs activation.
