The Effect of Different DAT Positive Intensities on Critically Ill Patients Receiving Red Blood Cell Suspension Infusion

Abstract:Objective To analyze and compare the effects of different positive intensities of direct antiglobulin test(DAT)on the efficacy and clinical outcomes of red blood cell suspension infusion in critically ill patients.Methods From March 2023 to February 2025,100 critically ill patients who required infusion of red blood cell suspension in Jilin People's Hospital were selected as the study sample.They were divided into 1+group(n=25),2+group(n=25),3+group(n=25),and 4+group(n=25)by DAT positive intensities.The effectiveness,adverse reactions,and clinical outcome indicators between four groups were compared.Results As DAT positive intensity increased,after infusion of red blood cell suspension,hemoglobin and red blood cell count decreased,while reticulocytes and total bilirubin increased.The ineffective treatment rate increased,the incidence of multiple organ dysfunction syndrome(MODS)increased,ICU stay was prolonged,and the 28 day mortality rate increased(P<0.05).Conclusion The positive intensity of DAT is significantly correlated with the risk of hemolysis and clinical outcomes in critically ill patients after transfusion of red blood cell suspension.When DAT positive intensity≥3+,the hemolysis process significantly intensifies,while the risk of ineffective infusion increases,and the incidence and mortality of MODS significantly increase.
