The Improvement Effect of Ipsilateral Stellate Ganglion Block on Intraoperative Hemodynamics,Postoperative Delirium and Pain in Elderly Patients Undergoing Thoracoscopic Lobectomy

Abstract:Objective To explore the improvement effect of ipsilateral stellate ganglion block(SGB)on intraoperative hemodynamics,postoperative delirium(POD)and pain in elderly patients undergoing thoracoscopic lobectomy.Methods Seventy-eight elderly lung cancer patients who underwent elective thoracoscopic lobectomy at Sanming First Hospital from December 2023 to October 2024 were randomly divided into an observation group(n=39)and a control group(n=39)using a stratified block randomization method.Thirty minutes before anesthesia induction,the observation group accepted SGB(0.3%ropivacaine 5 mL)on the surgical side under ultrasound guidance,while the control group was injected with an equal amount of physiological saline,and the rest of the anesthesia methods were the same.The intraoperative hemodynamic parameters and postoperative scores of visual analog scale(VAS)scores and 3-minute diagnostic confusion assessment method(3D-CAM),as well as POD incidence between two groups were compared.Results Finally,76 patients were included(38 cases in each group).During the operation,the heart rate,mean arterial pressure and oxygen saturation at T2 in the observation group were higher than those in the control group(P<0.05).After surgery,the VAS scores of the observation group were lower than those of the control group at all time points(P<0.05),and the total incidence of POD was lower than that of the control group(P<0.05).Conclusion Ultrasound-guided ipsilateral SGB can stabilize intraoperative hemodynamics,relieve postoperative pain and reduce the incidence of POD in elderly patients undergoing thoracoscopic lobectomy.
