Application Value of 24-hour Ambulatory Blood Pressure Monitoring in Differential Diagnosis of Masked Hypertension

Abstract:Objective To investigate the value of 24-hour ambulatory blood pressure monitoring in the differential diagnosis of masked hypertension(MH).Methods A total of 1,102 high-risk individuals with cardiovascular disease who were treated at the Chentangzhuang Street Community Health Service Center in Hexi District,Tianjin from November 2021 to November 2024 were selected as the research subjects.All individuals underwent clinic blood pressure measurement and 24-hour dynamic blood pressure monitoring.Analyze the diagnosis of hypertension in this population and the differences in blood pressure between different populations.Results Among the 1,102 subjects,538 had normal blood pressure,413 had true hypertension,31 had white coat hypertension,and 120 had masked hypertension.The detection rate of true hypertension and masked hypertension by 24-hour dynamic blood pressure monitoring is significantly higher than that of blood measurement in clinic(P<0.05).The systolic blood pressure in individuals with normal blood pressure and masked hypertension is significantly lower than that of individuals with true hypertension and white coat hypertension(P<0.05).The 24-hour dynamic monitoring blood pressure in individuals with normal blood pressure and white coat hypertension was significantly lower than that of individuals with true hypertension and masked hypertension(P<0.05).Conclusion The 24-hour dynamic blood pressure monitoring can effectively reduce the risk of missed diagnosis of masked hypertension,and also prevent its misdiagnosis as white coat hypertension.
