The Value of Quantitative Coronary Computed Tomography Angiography and Digital Subtraction Angiography in the Evaluation of the Coronary Artery Stenosis in Patients with Coronary Heart Disease

Abstract:Objective To explore the value of coronary computed tomographic angiography(CCTA)and digital subtraction angiography(DSA)in evaluating the degree of coronary artery stenosis in coronary heart disease.Methods A retrospective analysis was conducted on 100 patients with suspected coronary heart disease admitted to Linyi Central Hospital from October 2022 to October 2023.All patients underwent CCTA and DSA.Using DSA results as the gold standard,we evaluated the diagnostic efficacy of CCTA for coronary artery stenosis in coronary heart disease,analyzed the nature of plaques under different degrees of coronary artery stenosis,and further analyzed the differences in examination parameters among patients with mild,moderate,and severe stenosis.Results The diagnostic accuracy of CCTA for coronary artery stenosis was 93.00%,with a sensitivity of 96.63%and specificity of 63.64%.The number of segments diagnosed as mild,moderate,and severe stenosis by DSA were 157,70,and 36,respectively.While the number of segments diagnosed as mild,moderate,and severe stenosis by CCTA were 152,73,and 38,respectively.Kappa analysis showed a high degree of consistency between the two methods.The CCTA examination revealed that calcified plaques and mixed plaques are more common in the mild stenosis group,while non-calcified plaques are more prevalent in the moderate stenosis group.However,the proportions of the three plaque types do not significantly differ in the severe stenosis group.In different grades of stenosis,as the degree of stenosis worsens,minimal lumen area and minimum lumen diameter decreased,while the proportion of area stenosis and diameter stenosis increased(P<0.05).Conclusion Quantitative coronary CT angiography is of great value in evaluating the degree of coronary artery stenosis in coronary heart disease and can provide valuable reference for clinical diagnosis and treatment.
