The Influence of the Combination of the Healthy Side Lateral Running Position and Negative Pressure Aspiration in Flexible Ureteroscopy on the Stone Clearance Rate and Complications of Renal Pelvis and Upper Ureteral Calculi

Abstract:Objective To explore the impact of combining the healthy side lateral running position with negative pressure aspiration during Retrograde Intrarenal Surgery(RIRS)on the stone clearance rate and complications of renal pelvis stones and upper ureteral calculi(UUC).Methods A total of 84 patients with renal pelvis stones and UUC admitted to the Department of Urology of Anxi County Hospital from February 2023 to September 2024 were selected and randomly divided into a control group and an observation group,each consisting of 42 cases.Both groups underwent RIRS,with the control group receiving conventional lithotomy position plus negative pressure aspiration and the observation group receiving the healthy side lateral running position plus negative pressure aspiration.The conventional surgical indicators,stone clearance rates at 3 days and 1 month postoperatively,pain sensation(visual analogue scale,VAS),and complications(ureteral stone street and perforation,hematuria,urinary tract infection)were compared between the two groups.Results There was no significant difference in the intraoperative blood loss between the two groups(P>0.05).The operation time and the hospital stay required for the observation group were significantly shorter than those of the control group(P<0.05).The VAS score of the observation group was significantly lower than that of the control group(P<0.05).The stone clearance rate at 3 days after surgery in the observation group was 88.10%,which was significantly higher than 69.05%of the control group(P<0.05).There was no significant difference in the stone clearance rate between the two groups one month after surgery(P>0.05).The total incidence of complications in the observation group was 33.33%,which was significantly lower than 54.76%of the control group(P<0.05).Conclusion The combination of the healthy side lateral running position and negative pressure aspiration during RIRS for patients with renal pelvis stones and UUC had a shorter operation and hospitalization time,a higher stone clearance rate at 3 day postoperatively,less pain,and a lower complication rate.
