Abstract:Objective To investigate the effects of combination therapy of ulinastatin and ipratropium bromide on blood gas, respiratory mechanical parameters, and inflammatory response in patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD)with type Ⅱ respiratory failure. Methods Eighty patients with AECOPD with type Ⅱ respiratory failure who received treatment at the Yicheng Street Community Health Service Center in Yishui County from August 2021 to February 2024 were included. They were randomly divided into a control group and a study group, with 40 patients in each group. The control group received treatment with ipratropium bromide combined with budesonide, while the study group received treatment with ipratropium bromide combined with ustervastatin. The improvement of blood gas, respiratory mechanics parameters, and inflammatory response between two groups were compared. Results After one week of treatment, the oxygenation index, blood oxygen saturation, partial pressure of oxygen, and pH value in the study group were higher than those in the control group, while the partial pressure of carbon dioxide and blood lactate were lower than those in the control group(P<0.05).The peak airway pressure, inspiratory resistance, endogenous positive end expiratory pressure, and plateau pressure in the study group were lower than those in the control group(P<0.05).The interleukin-6,interleukin-8,high-sensitivity C-reactive protein, and tumor necrosis factor-α in the study group were lower than those in the control group(P<0.05).Conclusion The combination of ulinastatin and ipratropium bromide has significant therapeutic effects, could effectively promote the improvement of blood gas and respiratory mechanical parameters in AECOPD patients with type Ⅱ respiratory failure, and inhibite inflammatory reactions.
