The Predictive Value of Carotid Artery Intima-media Thickness and Stenosis Degree on the Degree of Coronary Artery Stenosis

Abstract:Objective To explore the predictive effect of carotid artery intima-media thickness(IMT)and stenosis degree on the degree of coronary artery stenosis.Methods A total of 300 patients diagnosed with coronary heart disease at Liaocheng Eye Hospital from June to December 2023 were selected.All patients were divided into three groups based on the degree of coronary artery stenosis:128 patients with mild coronary artery stenosis,95 patients with moderate coronary artery stenosis,and 77 patients with severe coronary artery stenosis,using digital subtraction angiography as the gold standard.All patients underwent carotid artery ultrasound examination to assess the IMT,plaque condition,and degree of stenosis of the carotid artery.Compare the IMT,plaque quantity,and area of carotid arteries in patients with different degrees of coronary artery stenosis.The efficacy of carotid artery IMT to determine severe coronary artery stenosis was evaluated through ROC curve.Analyze the consistency between the degree of coronary artery stenosis and the degree of carotid artery stenosis.Results As the degree of coronary artery stenosis worsens,the IMT of the carotid artery increases,the number and area of plaques increase,and there is a significant difference in the above indicators among three groups(P<0.05).ROC curve analysis showed that the area under the curve for determining severe coronary artery stenosis based on carotid artery IMT was 0.825(95%CI:0.425~1.009),with a cut-offpoint of 1.63 mm,sensitivity of 73.29%,and specificity of 90.82%.The degree of coronary artery stenosis is consistent with the degree of carotid artery stenosis.Conclusion Carotid artery ultrasound examination of the IMT,plaque condition,and lumen stenosis of the carotid artery in patients has a reliable indication of the degree of coronary artery stenosis,and can be used as a screening tool for coronary heart disease.
