Pathogen Distribution Characteristics and Risk Factors Analysis of Early Bacterial and Fungal Infections after Liver Transplantation

Abstract:Objective To explore the pathogen distribution characteristics and risk factors of early postoperative bacterial and fungal infections in liver transplant patients.Methods Two hundred liver transplant patients admitted to the Aviation General Hospital from January 2015 to December 2018 were selected,and the distribution of pathogens causing bacterial and fungal infections was statistically analyzed.Univariate and logistic multiple regression analyses were performed on the incidence of infections.Results Among the 200 liver transplant patients,143 cases(178 strains)developed bacterial or fungal infections,with an early infection rate of 71.50%.In those 48.31%of G-bacteria have an advantage in pathogenic bacteria,followed by 32.02%of G+cocci.The infection group had a higher proportion of patients with infection within 2 months before surgery,tracheal intubation≥48 hours after surgery,and reoperation compared to the non infection group(P<0.05).The results of logistic multivariate analysis showed that infection within 2 months before surgery(OR=3.674),postoperative tracheal intubation≥48 hours(OR=2.145),and reoperation(OR=1.895)were independent risk factors for bacterial and fungal infections in liver transplant patients(P<0.05).Conclusion Early postoperative bacterial and fungal infections are more common in liver transplant patients,with G-bacilli being the most common pathogen;Infection within 2 months before surgery,tracheal intubation≥48 hours after surgery,and reoperation can have an impact on the occurrence of infection.
