The Application Value of Color Ultrasound in the Diagnosis of Atypical Invasive Ductal Carcinoma of the Breast with BI-RADS Grade 4a Nodules Evaluated Preoperatively

Abstract:Objective To explore the application value of color ultrasound in the diagnosis of atypical invasive ductal carcinoma of the breast with BI-RADS grade 4a nodules evaluated preoperatively.Methods A study was conducted on 186 patients with breast nodules admitted to the First Affiliated Hospital of Guizhou University of Traditional Chinese Medicine from January 2020 to December 2023.According to postoperative pathological examination results,they were divided into a benign group of 124 cases and a malignant group of 62 cases.The various ultrasound features of preoperative ultrasound examination between two groups of patients were compared.The independent risk factors associated with the onset of malignant nodules were screened.The predictive model was establishd and its predictive performance for nodule properties was analyzed.Results Univariate analysis showed that the proportion of malignant group with irregular morphology,unclear boundaries,uneven edges,microcalcifications,and RI>0.7 was higher than that of benign group(P<0.05).Multivariate regression analysis showed that microcalcifications(OR=6.341),uneven edges(OR=5.518),and RI>0.7(OR=6.044)were independent risk factors for the development of malignant nodules(P<0.05).The multi-factor diagnostic model is logit(P)=1.847×X1(microcalcifications)+1.708×X2(uneven edges)+1.799×X3(RI>0.7)-1.879.The sensitivity,specificity,and area under the curve of this model for predicting nodule properties are 0.847,0.869,and 0.867,respectively.Conclusion Color ultrasound has good efficacy in preoperative evaluation of atypical invasive ductal carcinoma of the breast with BI-RADS grade 4a nodules,and the overall diagnostic performance of the diagnostic model is due to a single independent risk factor variable.
