The Improvement of Precision Management under Mobile Healthcare Mode Combined with Chain-Like Family Care Model to the Nutritional Status,Self-Care Ability and Hope Level of Hemodialysis Patients

Abstract：Objective To explore the improvement of precision management under mobile healthcare mode combined with chain like family care model to the nutritional status,self-care ability and hope level of hemodialysis patients.Methods Sixty hemodialysis patients admitted to Heping District Traditional Chinese Medicine Hospital in Tianjin from January 2021 to January 2023 were randomly divided into a control group and an observation group,with 30 patients in each group.The routine group received routine nursing,while the observation group received precision management under the mobile medical mode combined with a chain of family care additionally.The nutritional indicators,self-care ability,and hope level between two groups before and after intervention ere compared.Results After intervention,the level of blood phosphorus of observation group was lower than that of the control group,while the levels of prealbumin,body cell count,and lean body weight of observation group were higher than those of the control group(P＜0.05).The scores of self-care ability scale and Herth Hope Scale of the observation group were higher than those of the control group(P＜0.05).Conclusion The application of precision management under the mobile medical model combined with chain like family care model in hemodialysis patients can improve nutrition related indicators,enhance self-care ability and hope level.
