The Application Value of Integrated Neck-Brain Ultrasound to Evaluate the Carotid Artery Stenosis in Elderly Patients with Acute Cerebral Infarction

Abstract:Objective To analyze and explore the application value of integrated neck-brain ultrasound to evaluate the carotid artery stenosis in elderly patients with acute cerebral infarction.Methods A total of 168 elderly patients with acute cerebral infarction who underwent treatment in Zhangye People's Hospital Affiliated to Hexi University from December 2022 to December 2023 were selected.All patients underwent digital subtraction angiography(DSA),neck doppler ultrasound(CDU),and transcranial doppler ultrasound(TCD)examinations.The DSA results were used as the gold standard,and they were divided into non-stenosis group(n=31)and stenosis group(n=137).The stenosis group was divided into mild group(n=46),moderate group n=58),and severe group(n=33).The CDU and TCD parameters of each group were compared.The receiver operating characteristic(ROC)curves were applied to analyze the diagnostic efficacy of CDU combined with TCD for carotid artery stenosis.Results The levels of peak diastolic velocity,peak systolic velocity,pulsatility index,and resistance index in the stenosis group were significantly higher than those in the non-stenosis group(P<0.05).Subgroup analysis showed that the above indicators were ranked from high to low as severe group,moderate group,and mild group(P<0.05).ROC curve analysis showed that the sensitivity and specificity of CDU combined with TCD examination were high in the diagnosis of carotid artery stenosis and its severity,and the area under the ROC curve was significantly higher than that of single examination.Conclusion Integrated nec-brain ultrasound can effectively evaluate the severity of carotid artery stenosis and provide guidance for subsequent clinical diagnosis and treatment in elderly patients with acute cerebral infarction.
