The Application Effect of Community Diversified Nursing in Out of Hospital Management of Elderly Patients with Type 2 Diabetes Mellitus

Abstract:Objective To explore the influence of community diversified nursing on blood glucose control,self-management behavior compliance and self-efficacy of elderly patients with diabetes.Methods From October 2021 to October 2023,a total of 50 elderly patients with type 2 diabetes mellitus(T2DM)who established health records in Pudong Street Community Health Service Center of Beichen District in Tianjin were randomly divided into control group and management group,with 25 cases in each group.The control group adopted a conventional community nursing management model,while the management group adopted a diversified community nursing management model.The blood glucose control indicators,self-management behavior compliance,and self-efficacy were compared between two groups after 3 months of intervention.Results After 3 months of intervention,the frequency of hypoglycemia in the management group was significantly lower than that in the control group(P<0.05).The scores of dietary management,exercise management,medication management,and foot care in the management group were significantly higher than those in the control group(P<0.05).The scores of symptom management efficiency and disease common management efficiency in the management group were significantly higher than those of the control group(P<0.05).Conclusion Community diversified nursing can reduce hypoglycemic events during the treatment process of elderly T2DM patients,and improve their self-management behavior compliance and self-efficacy.
