Diagnostic Value of the Combination of RDW,D-dimer,and Lower Extremity Deep Venous Thrombosis in Elderly Patients with Pulmonary Thromboembolism

Abstract:Objective To analyze the relationship between red blood cell distribution width(RDW),D-dimer(D-D)and lower extremity deep venous thrombosis(LEDVT)and pulmonary thromboembolism(PTE)in elderly patients with pulmonary thromboembolism(PTE),and to explore the value of the combination of the three indexes in predicting PTE in the elderly.Methods The clinical data of 120 patients aged≥65 years with clinically suspected PTE admitted to Zhongshan Hospital Affiliated to Xiamen University during December 2018 to December 2020 were retrospectively analyzed and were divided into non-PTE group(CTPA-negative group)and PTE group(CTPA-positive group)according to their CT pulmonary angiography(CTPA)results.The correlation of RDW,D-D and LEDVT with PTE in the elderly was explored by univariate and multivariate analyses,and ROC curves for predicting PTE were plotted.Results The RDW,D-D and LEDVT in the elderly PTE group were significantly different with the non-PTE group(P<0.05).The multivariate logistic regression analysis showed that RDW,D-D and LEDVT were all risk factors for the occurrence of PTE in the elderly patients.The ROC curve showed that the area under the curve(AUC)of RDW,D-D and LEDVT were 0.723,0.753 and 0.754,respectively,and the combined AUC of the three indexes was 0.854(P<0.01).Conclusion RDW,D-D and LEDVT in elderly patients are significantly associated with the occurrence of PTE,and the combination of the three indicators has good diagnostic and predictive value,which is of clinical significance for the early identification of PTE in elderly patients.
