Clinical Diagnosis and Treatment of Gallstone Pancreatitis in Gestation Period

Abstract:Objective? To investigate the clinical experiences in diagnosis and treatment of biliary pancreatitis in gestation period. Methods? The clinical data of 24 pregnant patients with biliary pancreatitis from January, 2008 to January, 2015 were retrospectively analyzed. Results? In 24 patients, 12 cased with mild to moderate pregnancy induced biliary pancreatitis (MTMGPDP) (58.3%) cured by conservative treatment, 1 case underwent laparoscopic cholecystectomy combined with choledocholothotomy, lavage and drainage, and 10 cases performed SPDP. Of 10 SPDP patients, one case died of multiple organ failure, two cases improved after endoscope or intervention treatment, and seven cases cured after operation and minimally invasive debridement and drainage (2 cases underwent laparoscopic cholecystectomy combined with choledocholothotomy, lavage and drainage, 4 cases underwent laparoscopic cholecystectomy combined with choledocholothotomy, pancreatic necrosectomy lavage and drainage, and 1 case underwent laparotomy ). The fetal death happened in two cases (8.3%). Conclusion? In biliary pancreatitis patients during pregnancy, early dianosis, improvment of the the patients and fetal monitoring, and adoption of individual treatment according to disease degree and whether or not bile duct stone, are important for reducing maternal and fetal mortality.
