Effects of Lateral Perineal Resection and Natural Laceration on Pelvic Floor Function during Natural Delivery of Primipara

Abstract:Objective To investigate the effects of lateral perineal resection and natural laceration on pelvic floor function during natural delivery of primipara. Methods The clinical data of 607 parturients with vaginal delivery from June 2016 to December 2018 in New Century Women’s and Children’s Hospital and Chengdu Western Hospital were analyzed. According to whether or not perineal lateral resection was performed during delivery, all parturients were divided into perineal lateral resection group (n=302) and perineal protection group (n=305). Perineal lateral resection group was delivered by lateral perineal resection and transvaginal natural delivery was used in perineal protection group. The incidence of pelvic floor dysfunction and pelvic floor function were evaluated at 42 days after delivery between two groups. Results The incidence of perineal wound pain, perineal edema and perineal infection in lateral perineal resection group was significantly higher than perineal protection group (P<0.05).There were no significant difference in the incidence of dysuria, pressure urination incontinence and pelvic floor organ prolapse between the perineal lateral resection group and perineal protection group (P > 0.05). There were significant differences in the grade distribution of pelvic floor muscle strength in 42 days after delivery (P < 0.05). The normal rate of pelvic floor muscle strength in perineal lateral resection group was significantly lower than perineal protection group (P < 0.05). Conclusion The lateral perineal resection has great influence on pelvic floor function of primipara during natural delivery, which will increase the incidence of perineal wound pain, perineal edema and perineal infection. It should be strictly in accordance with surgical indications in clinical application.
