A Comparative Study on the Treatment of Unstable Distal Radius Fractures by Locking Plate Internal Fixation via Volar Approach and Dorsal Approach

Abstract:Objective To compare the effect of locking plate internal fixation through volar and dorsal approaches in the treatment of unstable distal radius fractures. Methods A total of 146 adult patients with unstable distal radius fractures admitted to our hospital from June 2014 to June 2018 were randomly divided into volar approach group and dorsal approach group, 73 cases in each group. The two groups were operated by volar approach and dorsal approach, respectively. The operation conditions,the wrist joint morphologyin 6 months after operation, and the wrist joint flexibilityin 1 year after operation were compared. Results The operation time of volar approach group was significantly shorter than that of dorsal approach group, the amount of bleeding during operation was significantly less than that of dorsal approach group, the incidence of complications was significantly lower than that of dorsal approach group, and the activity of dorsal extension was significantly higher than that of dorsal approach group (P<0.05). Conclusion Compared with dorsal approach in the treatment of unstable distal radius fractures, volar approach has certain advantages in operation time, incidence of complications and dorsal extension activity, and can be used as the preferred surgical approach in clinic.
