Application of Ultrasonography in Diagnosis of Pathological Nature of Ovarian Teratoma

Abstract:Objective? To investigate the ultrasonographic characteristics of ovarian teratoma and the difference between mature and immature teratoma based on blood flow. Methods A total of 137 patients with ovarian teratoma confirmed by laparotomy or laparoscopic surgery from 2013 to 2018 were selected retrospectively, consisting of 159 ovarian lesions and 204 teratoma. The ultrasonographic characteristics of ovarian teratoma were observed, and the ultrasonographic characteristics of mature teratoma and immature teratoma were compared. Results Of 137 cases, the teratoma was 44.53% on the left, 39.42% on the right and 16.06% on both sides. Of 159 ovarian lesions, single teratoma accounted for 73.58%, double teratoma accounted for 24.53%, and three teratoma accounted for 1.89%. In 204 teratoma, mature teratoma accounted for 73.04% and immature teratoma accounted for 26.96%. The blood flow peak speed and blood flow volume of mature teratoma were significantly higher than those of immature teratoma (P<0.05). The resistance index was significantly lower than that of immature teratoma (P<0.05). In 204 teratoma, the size is 2.0 cm × 1.8 cm × 2.3 cm ~ 4.7 cm × 5.2 cm × 4.6 cm. Texture: Cyst type accounted for 25.00%, substance type accounted for 23.53%, and mixed type accounted for 51.47%. Shape: circular or elliptical accounted for 79.41% and irregular shape accounted for 20.59%. Envelope: complete envelope accounted for 70.10% and incomplete envelope accounted for 29.90%. Ultrasonic images: lipid stratification sign accounted for 14.22 %, dough sign accounted for 25.00%, waterfall sign accounted for 15.20%, snowflake sign accounted for 8.82%, line sign accounted for 9.80%, wall nodules sign accounted for 18.14%, and messy structure accounted for 8.33%. Conclusion Ultrasound has high application value in the diagnosis of teratoma, and can be used for qualitative diagnosis according to the characteristics of blood flow sonogram.
