Diagnosis and management strategies of cholecysto-choledocholithiasis

Abstract:With the development of endoscopic technology and the deep understanding of the function of Oddi sphincter, the treatment mode of cholecysto-choledocholithiasis (CCL) is constantly changing with the guidance of enhanced recovery after surgery (ERAS) concept. For the diagnosis and treatment of CCL,the concept of "single stage" minimally invasive operation,"protecting the function of Oddi sphincter " and ERAS is gradually becoming the mainstream development direction. In order to achieve the purpose of cleaning the stones completely,preventing the complications and rapid recovery, laparoscopic techniques, ERCP and biliary microscopy should be applied according to the specific circumstances. Furthermore,traditional open cholecystectomy,choledocholithotomy and T tube drainage are still the optimal choices for the treatment of complex CCL cases and remain the basis for various minimally invasive biliary operation.
