Roles and status of salvage liver transplantation in the multidisciplinary treatment of hepatocellular carcinoma

Abstract:Liver resection (LR),though with unsatisfactory long-time outcome due to the high postoperative recurrence rate,is still the first-line treatments for hepatocellular carcinoma (HCC). Upfront liver transplantation (LT) is the gold standard in the treatment of patients with HCC and cirrhosis. Although the results of LT for HCC suggest it may be the preferable treatment for eligible patients,the worldwide shortage of organ donors limits its availability. Salvage liver transplantation (SLT) has recently been proposed and performed for those who undergo primary LR,followed by LT in the event of HCC recurrence or progressive liver deterioration,in order to take full advantage of LR and LT. As an important part of the multidisciplinary treatment of HCC, SLT has shown significant efficacy in not only small early-stage HCC but also large HCC which fit either the UCSF criteria or Hangzhou criteria. The way that SLT takes part in the multidisciplinary treatment of HCC is improving with the development of surgical techniques. Optimizing the role of SLT in the multidisciplinary treatment might be a potential way to improve the efficacy of treatment of HCC in China.
