Abstract:Gastric cancer has a relatively high incidence in China, and most of which are of advanced stages when diagnosed. A considerable proportion of the newly diagnosed advanced gastric cancer is unresectable because of severe local invasion and metastasis. Many clinical trials have demonstrated preoperative chemotherapy could improve prognosis of resectable local advanced gastric cancer, however, there is still no satisfactory strategy for unresectable local advanced gastric cancer. A modality of preoperative chemotherapy, which combined arterial with venous administration of chemotherapy drugs for unresectable local advanced gastric cancer has gotten satisfied effect. The clinical study showed an improved R0 resection rate, 1-year and 2-year survival, as compared to traditional chemotherapy with venous administration. It is an important and urgent issue to establish an effective strategy based on high level clinical evidence for improving prognosis of unresectable local advanced gastric cancer.
