Nuclear medicine imaging and therapy of gastroenteropancreatic neuroendocrine neoplasm

Abstract:Neuroendocrine neoplasms (NENs) comprise a heterogeneous group of neoplasms with very varying clinical expression. Imaging of NENs is challenging and the imaging needs in the various patients are diverse. The somatostatin receptors over expression on NENs cells,as compared to normal tissues,provide the molecular basis for many clinical applications of somatostatin analogues. Somatostatin receptor imaging, by 99mTc-HYNIC-Tyr3-OCT or PET with 68Ga-labelled somatostatin analogues, frequently detect additional lesions at staging and follow-up that are over-looked by CT or MRI. So far,somatostatin receptor imaging with SPECT remains the most widely available technique to assess tumour somatostatin receptor expression, and to evaluate a patient’s eligibility for treatment with unlabelled or radiolabelled somatostatin analogues. In the future, because of its higher sensitivity, PET with 68Ga-labelled somatostatin analogues is expected to replace somatostatin receptor imaging with SPECT.
