Choice of the Hepatic Vascular Occlusion for Resection of Large Liver Cancer with Cirrhosis.
Abstract:Objective To evaluate the outcomes resulting form the different choice of hepatic vascular occlusion for resection of large liver cancer with cirrhosis. Methods 21 cases of large liver cancer with cirrhosis were resected under 3 different hepatic vascular occlusions. The operational courses, the duration and type of hepatic vascular occlusion and the variation of postoperative liver function were analyzed. Results Of 21 patients,8 cases underwent the routine Pringle's maneuver, 11 cases underwent selective bemihepatic vascular occlusion, while the other 2 cases received comprehensive sequential hepatic vascular occlusion. All tumors were removed in intact and all patients were recovered uneventfully after operation. Conclusion The choice of hepatic vascular occlusion during bepatectomy of large liver cancer patient with cirrhosis should be made according to preoperative liver function tests and the results of images(CT or/and MRI), combined with the actual locations of the tumor during exploration. For the cases without or with mild cirrhosis, the routine Pringle's maneuver should be the first choice.
