Short-term clinical efficacy and safety analysis of endoscopic trans-gastric gallbladder-preserving cholecysto-lithotomy

Abstract：Objective To investigate the clinical efficacy and safety of endoscopic trans-gastric gallbladder-preserving cholecystolithotomy.Methods A retrospective analysis was conducted on 58 patients who underwent endoscopic natural orifice transgastric gallbladder-preserving cholecystolithotomy in the Department of Gastroenterology at Taian Central Hospital Affiliated to Qingdao University between April 2021 and February 2023.Clinical data included the success rate of the procedure,operation time,postoperative complications,length of hospital stay,and prognosis.Results The procedure was successfully completed in 56 patients(96.6％),with 2 cases of failure.Six patients had single gallstones,and 50 had multiple gallstones.The operation time ranged from 48 to 422 minutes,with a median time of 100.50 minutes and an average time of(120.83±73.35)minutes.Six patients experienced mild postoperative abdominal pain,which reslieved within 3 days.One patient(1.7％)developed postoperative pneumothorax,who received ICU treatment for 4 days before being transferred back to our department and eventually was discharged in improved condition.A 1-year follow-up revealed gallstone recurrence in 5 patients(8.6％).Conclusion Endoscopic trans-gastric gallbladder-preserving cholecystolithotomy is safe and effective,with minimal discomfort and rapid postoperative recovery,which meets the patients'desire for preserving gallbladder function.
