Feasibility study of the Selinexor-Melphalan combined conditioning regimen for autologous stem cell transplan-tation in multiple myeloma

Abstract:Objective To explore the feasibility of the Selinexor-Melphalan combined conditioning regimen for autologous stem cell transplantation in multiple myeloma.Methods From April 2023 to June 2024,11 patients diagnosed with MM who underwent ASCT enrolled in Tongji Hospital,Tongji Medical College,Huazhong University of Science and Technology.The conditioning regimen included Selinexor 60 mg on days-10 and-3 prior to hematopoietic stem cell infusion and a standard or reduced dose of Melphalan on days-2 before infusion.The primary endpoints were the complete response(CR)rate and safety,while secondary endpoints included progression-free survival(PFS),measurable residual disease(MRD)negativity rate,overall survival(OS),and improvement in renal function.Results The median time of neutrophil and platelet engraftment was 13(12-13)d and 15(12-16)d,respectively.Of the 11 patients,Nine patients achieved CR one month after ASCT,while two were still PR.One patient experienced disease progression three months post-ASCT,with a CR rate and MRD-negative rate of 72.7%three months after transplantation.Treatment-related adverse events included hyponatremia(90.9%),nausea(81.8%),vomiting(54.5%),hypokalemia(45.5%),febrile neutropenia(45.5%),oral mucositis(45.5%),and pneumonia(9.1%).All patients were alive at the end of the follow-up,with median PFS and OS not yet reached.Conclusion The Selinexor-Melphalan combined conditioning regimen for MM patients undergoing ASCT is feasible and tolerable.
