Predictive value of peripheral systolic blood pressure and pulse pressure for new-onset carotid plaque in community population

Abstract:Objective To investigate the predictive value of peripheral systolic blood pressure (SBP) and pulse pressure (PP) for new-onset carotid plaque in a Chinese community-based population. Methods A total of 748 participants without carotid plaque at baseline from an atherosclerosis cohort with 2.30-year follow-up in Beijing, China, were included in our analysis. The inclusion criteria were: complrtr data, no carotid plaque at baseline, completing the follow-up of 2004. Logistic regression models were used to examine the association between SBP, PP and new-onset carotid plaque. Results Subjects were averagedly (51.5±4.9) years old, and 28.0% (n=213) were male; 34.7% (n=264) had hypertension, 15.4% (n=117) had diabetes, and 67.2% (n=511) had dyslipidemia. After a median of 2.30 years of follow-up, new-onset carotid plaque ratio was 30.9% (n=235). Every 10 mmHg increase of SBP and PP was associated with 14% and 27% increase of carotid plaque after adjustment for gender, age, body mass index, eGFR, current smoking and drinking status, dyslipidemia, diabetes mellitus, lipid-lowering medications and hypoglycemic medications. When SBP and PP were forced into the model simultaneously, only PP showed significant correlation with new-onset plaque (for per 10mmHg increase of PP, OR=1.27, 95% CI 1.00-1.62; for per 10 mmHg increase of SBP, OR=0.99, 95% CI 0.83-1.19). Subgroup analysis showed that compared with the lowest tertile group, PP in the highest tertile group had significant higher risk of new-onset carotid plaque. Conclusion Both SBP and PP are associated with increased risk of new-onset carotid plaque, whereas the predictive value of PP for new-onset carotid plaque is better than SBP.
