Clinical features of intractable insulin allergy and the treatment strategy

Abstract:Objective To investigate the clinical features of and effective therapy for intractable insulin allergy.Methods Seven hospitalized patients with intractable insulin allergy in Peking Union Medical College Hospital from 2014 to 2016 were retrospectively reviewed for clinical features,results of skin prick tests and insulin intradermal injection tests,long-term therapy and glycemic control.Results Three in 7 patients were diagnosed with type Ⅰ hypersensitivity;and they presented persistent symptoms of insulin allergy again back in MDI mode after successful desensitization by continuous subcutaneous insulin infusion.The other 4 patients were diagnosed with type Ⅳ hypersensitivity with various symptoms and different individualized desensitization therapy.Conclusions Intractable insulin allergy includes type Ⅰ hypersensitivity with low threshold of allergy and type Ⅳ sensitivity with various symptoms.The desensitization therapy and long-term therapy should be designed individually.
