Chest ultrasonography for assessment of extravascular lung water in patients with acute respiratory distress syndrome

Abstract:Objective To study compare the sensitivity and specificity of chest ultrasound.Methods Twenty-one patients with a clinical diagnosis of ARDS consecutively admitted to the Intensive Care unit of FuXing Hospital underwent CT in the first 24h admission,and their general information was recorded.The ARDS patients were performed beside chest ultrasound,chest radiology,PiCCO monitoring respectively on the first,second and third day.Fifteen control cases without ARDS were performed chest CT and ultrasound,chest radiology in the first 24 h.Result Significant positive linear correlations were found between comet score and EVLW determined by the PiCCO system (r=0.927,P<0.01),and comet score was positively related to chest X-ray score (r=-0.720,P<0.01).The area under the ROC curve of ELVW by chest radiography was respectively 0.924,0.732(P<0.01,P=0.019),the cut-off value was 12.5 and 14.0 respectively,and sensitivity was 0.857,0.619,and specificity was 0.6,0.667.Conclusion At the bedside,lung ultrasonography is highly sensitive,specific,and reproducible for assessing EVLW in patients with ARDS quantitatively and qualitatively.
