Clinical outcomes of inspiratory muscle training and non-invasive positive pressure ventilation for severe chronic obstructive pulmonary disease

Abstract:Objective To investigate the effects ofinspiratory muscle training and non-invasive positive pressure ventilation in patients with severe chronic obstructive pulmonary disease (COPD).Methods A total of 88 patients with stable COPD were randomly assigned into four groups,to continue oxygen therapy (control group,n=22),to receive inspiratory muscle training add non-invasive positive pressure ventilation(IMT-NPPV,n=22),inspiratory muscle training (IMT,n=22) or noninvasive positive pressure ventilation (NPPV,n=22) for at least 8 weeks.The outcomes were evaluated quality of life(SRI),MIP,MEP,MRC and lung function.Results Compared to baseline value,SRI score increased statistically after 8 weeks in group IMT-NPPV,group IMT and group NPPV,with significant/△SRI difference being observed between group IMT-NPPV and group IMT as well as NPPV(all P< 0.05).MRC scale declined statistically in group IMT-NPPV,group IMT and group NPPV,with significant/△MRC difference being observed between group IMT-NPPV and group IMT,NPPV(all P< 0.05).After 8 weeks,6MWD increased statistically in group IMT-NPPV,IMT andNPPV(all P< 0.05).△ 6MWD was statistically different between group IMT-NPPV group IMT and NPPV(all P< 0.05).MIP and MEP changed statistically in group IMT-NPPV,IMT(all P< 0.05),with significant difference between group IMT-NPPV and group IMTNPPV as well as group IMT(P<0.05).These were no significant changes in pulmonary function parameters after 8 weeks compared to baseline values in all groups (all P>0.05) Conclusion Inspiratory muscle training and non-invasive positive pressure ventilation is better than inspiratory muscle training or non-invasive positive pressure ventilation.
