Blood pressure management for aging patients
LIN Zhan-yi
Abstract:Elderly patients with hypertension have increased quickly, which has clinical features of high systolic blood pressure, large blood pressure fluctuations, abnormal circadian rhythm,white-coat hypertension and pseudohypertension, etc.Elderly patients with blood pressure more than 150/90 mmHg will receive anti-hypertensive therapy.Target blood pressure under 140/90 mmHg is suitable when patients can tolerate. But it should not be lower than 130/60 mmHg for very older patients. The anti-hypertensive treatment involves non-drug therapy and drug therapy, and the five guideline-recommended drug classes are commonly used.Isolated systolic hypertension, morning hypertension and sickly coexistence are common types in elderly patients with hypertension.Meanwhile refractory hypertension is not rare.We need to pay attention to the merger with orthostatic blood pressure variability, postprandial hypotension and other special problems of elderly hypertension.
