Damege of masked hypertension on kidney in type 2 diabetes patients

Abstract:Objective To evaluate the impact of masked hypertension (MH) in type 2 diabetic patients on kidney damage.Methods Totally 131 hospitalized patients with type 2 diabetic mellitus (T2DM) who accepted 24-h ambulatory blood pressure monitoring (ABPM) were enrolled,divided into MH group (n=70) and normotensive (NT) group (n=61) according to ABPM results.All patients underwent 24-h urine protein quantity test,and relevant data were compared in MH and NT patients.Results The incidence of MH was identified in 53% in hospitalized diabetic patients in the study.Masked hypertensive patients and normotensive patients were not different in terms of age,diabetic duration,fasting blood-glucose (FPG),glycosylated hemoglobin (HbA1c),lipid profiles,smoking status,drinking history and body fat rate.BMI of MH patients were significantly higher than those of NT group (24.72±3.12 vs 23.57±+2.86,P=0.031).The incidence of diabetic microalbuminuria (MAU) (24.3% vs 4.9%,P=0.002) was also increased in masked hypertensive patients.Regression analysis indicated that 24h-mALB was related to daytime systolic blood pressure measured by ABPM and diabetic duration (P=0.021,0.024).Conclusion Type 2 diabetic patients with MH have increased risk in kidney microvascular complications.
