Day-time continuous renal replacement therapy in treatment of acute severe hyponatremia

Abstract:Objective To evaluate the therapeutic effect of day-time continuous renal replacement therapy (CRRT) in patients with acute severe hyponatremia.Methods Thirteen patients with acute severe hyponatremia (serum sodium concentration<115 mmol/L before CRRT) were enrolled.The day-time CRRT was conducted with Baxter BM25 and HF1200 hemofilter and low molecular weight heparin sodium was used for anticoagulation.The replacement solution was infused at the rate of 4 L/h by a pre-dilution route,and treatment duration was set for 8 hours everyday.Results The average treatment time of CRRT was (3.1±1.0) d.All patients tolerated CRRT well,of which twelve patients survived and one patient died due to ARDS.The serum sodium concentration increased from(112.5±2.4) mmol/L at the initiation of CRRT to(139.6±2.8) mmol/L after 3 days'treatment and the serum osmolarity was corrected concurrently.After CRRT,the Glasgow scores improved significantly and no severe nervous system complication occurred in all of the patients.Conclusion Day-time CRRT is safe and effective in treating acute severe hyponatremia,and may be considered as a treatment option.
