Related factors of pathogens distribution of community acquired pneumonia
Abstract:Objective To study the related factors of pathogens distribution of community acquired pneumonia.Methods All 320 patients were recorded information of onset, at the time of enrollment. Sputum cultures were performed. The first and convalescent serum are collected for MP, CP and LP antibody test. Statistical analyses are performed using the SPSS 12.0 software. Results A total of 186 bacterial pathogens were isolated from 320 patients. SP(25.8% ) was the most fre-quent isolate,followed by HI(21. 5% ) ,KP(9.7% ) and Staphylococcus aureus (4.3% ). Serological detection of atypi-cal pathogens indicate that 130(40.6% ) patients were confirmed of MP infection,46 (14.4% ) of CP infection and 13 (4.1% ) of LP infection. Bacteria and atypical pathogens mixed infection was common in this study. 30.8% MP and 21.7% CP were concumently infected with bacterial pathogens. The patients who were &65 years old,having COPD his-tory purulent sputum,leukocytosis > 10×10~9 cells/L,and PORT score >70 were easier to isolate bacterial pathogens. SP was more common in patients with purulent sputum,leukocytosis>10×10~9 cells/L,and PORT score >70.But MP was more common in patients <65 years old.non COPD,not having purulent sputum,leukocytosis 4 ~ 10×10~9 cells/L,and PORT score≤70.The difference was significant between the groups (P<0.05). Conclusion The top three bacteria pathogen of CAP in China is SP, HI and KP. There was a high prevalence of atypical pathogens of CAP in China. Patients' some symptoms,disease history and lab investigation can be used to diagnose special pathogens infection.
