Analysis of risk factors for premature delivery and abortion in subchorionic hematoma

Abstract:Objective To study the risk factors for premature delivery and abortion in subchorionic hematoma.Methods Retrospectively analyze the influencing factors of adverse pregnancy outcomes such as premature delivery and abortion in singleton pregnancy patients diagnosed with subchorionic hematoma in Qilu Hospital of Shandong University(Qingd-ao)from June 2014 to June 2022.Results Of the 78 patients who met the inclusion criteria,8 were less than 27 gesta-tional weeks.Hematoma volume in 57 cases(A1)was less than 20cm3,12 cases(A2)were in scope of 20-30cm3,and 9 cases(A3)were more than 30cm3.The volume ratios of hematoma to gestational sac were less than 10%in 11 cases(B1),>10%-25%in 25 cases(B2),>25%-50%in 22 cases(B3),and more than 50%in 20 cases(B4).The preterm birth rate was higher in women diagnosed before 8 weeks of gestation(P<0.05).Compared to group A1,the placental adhesion rate was higher in group A3(P<0.05).Compared to the other three groups,group B4 had lower weeks of termination,term birth rate,live birth rate and neonatal weight,and higher abortion rate(P<0.05);Multivariate logistic regression analysis showed that advanced age and hematoma to gestational sac ratio ≥25%were risk factors for miscarriage,and his-tory of intrauterine operation was a risk factor for preterm birth.Conclusions Advanced age and hematoma to gestation-al sac volume ratio≥25%are risk factors for abortion in patients with subchorionic hematoma.The history of intrauterine operation is a high risk factor for preterm birth.The preterm birth rate is higher in gestational weeks less than 8 and pla-cental adhesion rateis higher in hematoma volume more than 30cm3.
