En caul vaginal delivery of the second preterm infant in twin pregnancy

Abstract:Objective To describe the vaginal delivery of second fetus in preterm birth of twin pregnancy within intact amniotic sac, and to compare the immediate neonatl outcomes of the second fetus and stages of labor with those delivered with non-intact amniotic sac.Methods Totally 181 preterm twins were selected from May, 2016 to Feb, 2018 in the Third Affiliated Hospital of Guangzhou Medical University, and were randomly divided into'En caul'vaginal delivery group and control group. The relationship between gestational week and birth weight of second fetus and the success rate of"En caul"births was evaluated. Apgar scores, umbilical cord artery blood gas p H, lactate, hemoglobin and birth injury of the second fetus also the stages of labor were noted.Results (1) The birth weight of second fetus of successful En caul vaginal delivery group was (1343.5±409.1) g, significantly lower than that of failure group (P<0.05), and gestational week of successful group was (30.8±2.4) weeks, apparently lower than that of failure group (P<0.05). (2) Compared with the failure group[ (5.4±0.3) mmol/L], the cord artery blood lactate of successful group[ (4.0±0.9) mmol/L]decreased significantly (P<0.05). (3) The cord artery blood hemoglobin of successful group showed no marked decrease (P>0.05). (4) In the failure group, 3 cases (15.8%) of the cerphalic presentation had intracranial hemorrhage, 2 cases (6.5%) of the breech presentation had bruising injuries, both significantly higher than those of successful group (P<0.05) (0/11 with the cerphalic presentation showed intracranial hemorrhage and 0/30 with the breech presentation presented bruising injuries). (5) The third stage of labor of successful group was significantly shorter than the failure group (P<0.05).Conclusion (1) The success rate of En caul vaginal delivery apparently increases in the cases of small gestational week and lower birth weight of the second fetus, and en caul vaginal delivery can improve the birth outcomes of the second fetus.
