Analysis of the relationship between the risk of preterm delivery in singleton pregnancy and in vitro fertilization and embryo transfer

Abstract:Objective To investigate the relationship of the risk of premature delivery among singleton pregnancies with IVF-ET.Methods A group of 563 women who conceived by IVF-ET from January 2012 to January 2013 in obstetric ward of Peking University Third Hospital were chosen as the study group (IVF-ET group),and 1126 women who gave birth during the same period of time in the same hospital were chosen as the case-control group (nature conceived group or NC group) according to their mean maternal age,gravidity and parity.The IVF-ET group was divided into female-factor (n=369),the"pure" male-factor infertility subgroups (n=150) and the unexplained infertility subgroup (n=44).The pregnancy outcomes of these two groups were compared.Results The preterm delivery rate in IVF-ET group was significantly higher than control group (11.90% vs.6.93%,P<0.05) IVF-ET singleton pregnancies were associated with an increased risk of pregnancy-induced complications,such as gestationa mellitus diabetes,premature rupture of membranes,previa placenta and postpartum hemorrhages,with statistical significance (P<0.05).The perinatal mean gestational week was lower than NC group [(38.64±2.16) weeks vs.(39.12±-1.73) weeks,P<0.05],while low birth weight rate was higher than NC group (8.34% vs.4.35%,P<0.05).Within IVF-ET group,the incidence rates of prematurity,PROM,and postpartum hemorrhages in female-factor infertility group were significantly higher than the"pure"male infertility group (P<0.05).The incidence rates of gestational mellitus diabetes,premature rupture of membranes,previa placenta and postpartum hemorrhages in female-factor infertility group were significantly higher than NC group.The perinatal mean gestational week was lower than "pure" male-factor infertility group and NC group,while low birth weight rate were higher than NC group (P<0.05).Conclusion The increased rate of prematurity of singleton IVF-ET pregnancies is related to the diseases related to infertility and higher rate of obstetric complication in female-factor infertility women.
