Treatment of cervical intraepithelial neoplusia using the loop electrosurgical excision procedure

Abstract：Objective To study the effect of LEEP to treat CIN. Methods A retrospective study was conducted on LEEP in 452 cases of CIN at the First Affiliated Hospital of Chong Qing Medical University between Jan. 2004 and May 2007. The operative time,intraoperative blood loss, colposcopic site-specific biopsies and follow-up data were retrospectively analyzed. Results (1) The mean operative time was 5 ～ 20 min, the operative blood loss was 14.6 mL, and the Reaction was light. (2) Histologic findings revealed that 29.20% (132/452) CIN was grade lowed, 59.73% (270/452) CIN grade was remained the same and 11.06% (50/452)CIN grade was gone up, of which five eases of CIN1 was canfirmed to CIN3 ,compared with biopsy-confirmed CIN.25 positive margins were found, and hysterectomy was performed in 8 of 25 cases with positive margins, so did 3 secondary LEEP. (3)The cure rate was 92.14% ;The residual rate was 7.86% ;the recurrence rate was 0.48% when all patients were followed up regularly by Aug 2007. (4) 8 of 18 cases (44.44%) with positive magins recurred and 25 out of 402 cases (6.22%) with negative margins recurred (Fisher test, P = 0.0000). Conclusion LEEP is a kind of safe and effective method for treatment of CIN. Margin involved CIN is one of the factors affecting recurrence.
