Abstract:Complex intracranial aneurysms are refractory cerebrovascular lesions. In the context of microsurgical clipping or endovascular treatment, complex intracranial aneurysms are defined as intracranial aneurysms that cannot be safely and effectively managed using conventional methods due to factors such as morphology, size, aneurysm neck structure, and the intricate relationship between the parent artery and its branch vessels. In recent years, with advancements in microsurgical techniques and intraoperative monitoring, the application of cerebral revascularization for the treatment of complex intracranial aneurysms has become increasingly mature, demonstrating overall safety and efficacy. This article reviewed the classification of bypass methods, indications, selection of donor and recipient vessels, preoperative assessment, intraoperative monitoring, and clinical effectiveness aiming to provide a comprehensive reference for clinicians to formulate individualized treatment strategies for intracranial complex aneurysms.
