A case report of mechanical thrombectomy for basilar artery occlusion following dual-access retrograde vertebral artery revascularization via the ascending cervical artery

Abstract:Acute basilar artery occlusion is a cerebrovascular disease with high mortality and disability rates.Early recanalization of the occluded vessel is the cornerstone of treatment.In patients with tandem lesions involving the dominant vertebral artery,revascularization of the occluded vertebral artery is the priority to establish access for subsequent basilar artery thrombectomy.When antegrade recanalization of the vertebral artery is challenging,a retrograde approach via collateral circulation may serve as an alternative strategy.This article presented a case of acute basilar artery occlusion with failed initial antegrade recanalization of the occluded left dominant vertebral artery.Successful retrograde revascularization was subsequently achieved via the anastomosis between the ascending cervical artery and the left vertebral artery,followed by antegrade guide catheter placement through the left radial artery approach to perform thrombectomy.The occluded vessel achieved favorable recanalization,and the patient's 90-day follow-up modified Rankin scale score was 1.
