Primary epithelial-myoepithelial carcinoma in the sellar region:a case report

Abstract:Epithelial-myoepithelial carcinoma(EMC)is a rare low-grade malignant epithelial neoplasm,and EMC in the sellar region is particularly uncommon.Due to the lack of specific clinical and radiological manifestations,preoperative diagnosis of EMC is challenging and misdiagnosis is prone to occur.This article reports a case of primary EMC in the sellar region in a 28-year-old male who presented with blurred vision in both eyes for over 5 months.MRI revealed a lobulated mass in the sellar region,ap-proximately 3.5 cm×3.4 cm×3.0 cm in size,showing T1-weighted contrast enhancement,with compression and displacement of the optic chiasm.A sellar tumor was suspected.After completing preoperative preparations,the sellar lesion was totally resected via a pterional approach.Postoperative pathological diagnosis confirmed EMC.Six months postoperatively,recurrence of the sellar tumor occurred.PET/CT scan showed no systemic metastases,and the patient subsequently underwent Gamma Knife radiosurgery.Follow-up contrast-enhanced MRI of the brain six months after Gamma Knife treatment showed no abnormal signal in the sellar region.This case suggests that EMC should be considered in the differential diagnosis of sellar lesions,and early detection and timely surgery are crucial.
