A case of brain abscess following endovascular embolization of an intracranial aneurysm

Abstract:Intracranial abscess secondary to endovascular embolization of intracranial aneurysms is clinically rare.This article reports a case of a 68-year-old male admitted to the hospital due to severe headache for one day.Head CT revealed subarachnoid hemorrhage,and DSA identified an anterior communicating artery aneurysm.Emergency coil embolization was performed,but the aneurysm was not completely occluded,with residual neck remaining.On the 4th day post-surgery,the pa-tient developed a fever,with a maximum temperature of 38.9℃.Blood tests and cerebrospinal fluid analysis diagnosed intracra-nial infection,and anti-infective treatment with vancomycin+meropenem was initiated.On the 21st day post-surgery,a follow-up head MRI revealed bilateral frontal lobe abscesses.The patient continued anti-infective treatment with vancomycin+meropenem and underwent lumbar cisternal drainage for cerebrospinal fluid for 6 weeks,eventually recovering and being discharged.This case suggests that close monitoring is essential after partial embolization of a ruptured intracranial aneurysm.Once signs of in-tracranial infection are detected,prompt diagnosis and treatment should be implemented to improve patient outcomes.
