Research progress on the pathogenesis and drug therapy of chronic subdural hematoma

Abstract:Chronic subdural hematoma,one of the most common neurosurgical conditions,is characterized by insid-ious onset and slow progression,predominantly affecting the elderly.With the accelerating aging population and the increasing use of anticoagulant and antiplatelet medications,its incidence continues to rise.Although burr hole drainage remains the pri-mary treatment,the high postoperative recurrence rate persists as a clinical challenge,underscoring the significance of exploring safe and effective nonsurgical strategies.Recent research has focused on the pathophysiological mechanisms of chronic subdural hematoma,proposing core hypotheses such as local inflammatory response,abnormal angiogenesis,and hyperfibrinolysis,which have informed the development of pharmacological therapies including corticosteroids,atorvastatin,and tranexamic acid.This article systematically reviews advances in pathogenesis of chronic subdural hematoma,outlines key pathological pathways,and evaluates the efficacy and challenges of current drug therapies,aiming to provide direction for future research.
