Efficacy of posterior median approach via microchannel for microscopic resection of spinal intramedullary cavernous hemangiomas

Abstract:Objective To evaluate the efficacy of posterior median approach via microchannel for microscopic resection of spinal intramedullary cavernous hemangiomas.Methods The clinical data of 26 patients with spinal intramedullary cavernous hemangiomas surgically treated between January 2020 and June 2024 were retrospectively analyzed.All patients underwent microscopic resection via posterior median approach with microchannel technique.Results Hemilaminectomy was performed in 18 cases and total laminectomy in 8 cases.Incision length was(3.5±0.5)cm,and hospital stay was(5.0±2.0)days.Gross total resection under microscopy was achieved in all 26 cases(100.0%)without intraoperative complications.Postoperative complications occurred in 5 cases(19.23%).Modified McCormick Scale scores at 3 months postoperatively[(2.7±1.0)points]were significantly lower than preoperative scores[(3.4±1.1)points;P=0.026].At 6-month follow-up,neurological function improved in 17 cases(65.4%)and remained stable in 9(34.6%).No signs of kyphosis or recurrence were observed.Conclusion Microscopic resection of spinal intramedullary cavernous hemangiomas via posterior median approach with microchannel technique is feasible,demonstrating safe and reliable therapeutic outcomes.
